The Planning Worksheet was developed to be used at the
Individual Education Plan (IEP) meeting. In this parent course
we are breaking the worksheet into sections and asking you to focus
on one section at a time. Accessing Adult Services asks for rating
on two statements.
Your son or daughter can identify:
- Public and private agencies that provide support services to college
students with disabilities
- Application procedures for these support services
Using a three
point scale, you will be asked to rate each statement.
To help you prepare for the next IEP meeting use the
Adult Services
Section. Following your rating
of each question, there is room to record your questions, concerns
or thoughts, for the IEP committee. Copy and take it with you to
the meeting. Even if you are not meeting with school staff, complete
the rating form to help you decide where to start with Post-ITT
Guidance Activities.

1=no
knowledge or ability,
2= some knowledge or ability,
3=high level of
knowledge or ability
