The Planning Worksheet is a simple rating form to determine those Guidance Activities that are important for your son or daughter. It was developed to be used at the Individual Education Plan (IEP) meeting. It works best if all IEP participants, especially the student, complete the ratings on separate forms. In this parent course the Worksheet is broken down into sections so you can focus on just one topic at a time.

The Assistive Technology section of the Worksheet has 4 statements. On a three point scale, you are asked to rate your son/daughter's understanding of:

  • Assistive technology and how it can be used to accommodate learning

  • How everyday items can be used as assistive technology

  • Which high technologies can assist him/her in learning

  • The assistive technology he/she currently uses

To help prepare for the next IEP meeting use the Assistive Technology section of the Planning Worksheet. After you have rated each statement, there is room to note questions or thoughts, for discussion at the IEP meeting (remember to take a copy!). If you are not meeting soon, complete the rating form anyway to help you decide where to start with the Guidance Activities.
