As you have learned, the Planning
Worksheet was developed to be used at the Individual Education
Plan (IEP) meeting. It asks you, your son or daughter, and other participants
to rate statements on a three point rating scale. Here are the six
statements for this section.
Your son or daughter can identify:
- The impacts of the law on the roles of students, teacher, and school
- The requirements and demands of college coursework
- The range of support services through DS and other campus resources
- Differences in support services among colleges
- Their need for disability service in college and how to establish eligibility
To help you prepare for the next IEP meeting, use the Disability Services Section of the
Planning Worksheet.
Following
your rating of each question, there is room to record your questions,
concerns or thoughts, for the IEP committee. Copy and take with
you to the meeting. Even if you are not meeting with school staff,
complete the rating form to help you decide where to start with
Post-ITT Guidance Activities.

3=high level of knowledge or ability.
