The Post-ITT Planning WorksheetTo insure that students are part of transition planning, the IDEA requires that they participate in their Individualized Educational Plan (IEP) once they are 14 years of age. The Post-ITT Planning Worksheet provides a basis for the discussion of issues related to going to college. It then can be used as a roadmap for deciding which Post-ITT Guidance Activities should be used to reach the outcome of going to college.Prior to the IEP meeting, the student, parent, and teacher rate the student's knowledge and skill on a three point scale. At the meeting, participants explore and reconcile the ratings from their different perspectives. This discussion then results in a plan and selection of the appropriate Post-ITT Activities. The IEP committee meeting is the best time for this discussion because the key people are present. For students with disabilities who do not have an IEP committee (such as those with a 504 accommodation plan) this process can be modified as necessary. ResourcesThe Post-ITT Web site provides many of the resources needed to undertake the Guidance Activities. The site has all of the Guidance Activities; information on specific topics such as Disability Law, Disability Services and links to other relevant Web-based resources.Postitt.org has resources for students, parents and professionals such as this Web course, the Senior Checklist and downloadable activities. Because there are so many potentially valuable resources, Post-ITT has a search engine to expedite locating relevant resources. Word-Ware generously sponsors the cost of the database that contains the Post-ITT resources. Post-ITT also relies on the search capabilities of Google™. Student EvaluationEach Guidance Activity has a set of questions that you can use to evaluate progress. These are located in the Post-ITT Teacher Manual and in this training module. By using these questions and maintaining a record of progress made, you will be able to adjust your pace of instruction and supplement the Activities as necessary. The Teacher Manual makes suggestions for enrichment or alternative activities.The IEP team should evaluate progress periodically. The Planning Worksheet will be a useful tool to evaluate progress toward the goal of preparing for college.
